Page Text ---------- Mango Tango Art Gallery YOUR TELEPHONE BILL AND HOW TO PAY IT JUDY KING It is good practice to look over your telephone bill carefully each month. If you find an item you do not understand, or an item you question, please call our Business Office promptly. (See pages 1 & 2 for office numbers and locations.) Our Service Representative will be happy to help you. WHEN IS THE BILL DUE? Bills are due when rendered. Accounts that remain unpaid fifteen (15) days after the bills are mailed are subject to a late payment charge of $2.00. DISCONNECTION & RECONNECTION FOR NON-PAYMENT If the account remains unpaid ten (10) days after the delinquent notice is mailed, service will be disconnected. In the event of an authorized disconnection, but not removal of facilities, service will be restored upon payment of the total amount due plus a reconnection charge of $15.00. TERMINATION OF SERVICE Telephone facilities and service may be removed five (5) days after authorized disconnection in which event re-establishment of service will be upon the basis of a new service application as well as installation and deposit fees. FINAL BILLS & DEPOSIT REFUND Upon removal of telephone service, either for non-payment or by the customer s request, INNOVATIVE TELEPHONE will issue a final bill. Final bills are due upon receipt. INNOVATIVE TELEPHONE shall refund the balance of the customer s deposit plus accrued interest (if any), provided there are no outstanding balances owed to the company. 32
